HFS Employee Dependent Details

This form is for the purpose of collecting Employee Dependent Details for purpose of updating HRMS records and for future Health Insurance coverage.

Please read the instructions carefully before you proceed:

1. Review your dependent details in Darwinbox -> Profile - > Personal Details -> Dependents (Spouse, Parents and Children).

2. In case there are any changes or updates to the dependents list, choose - UPDATE DEPENDENTS and kindly fill the form with complete dependent details.
3. If the dependent details in Darwinbox are accurate and need not be changed, please choose the option as provided - NO CHANGE.
4. Ensure you fill up the Name and Date of Birth correctly as per Aadhaar card as this will directly impact future claims.
